A 75 year-old woman developed the acute onset of ataxia, diplopia and confusion. |
| Multiple Brainstem and Cerebellar Infarctions: Axial CT scans 3 days after presentation. Along with the infarctions present in the cerebellum and brainstem, note the dilatation of the lateral ventricles, indicating early obstructive, non-communicating hydrocephalus. This is a known, potentially fatal complication of posterior circulation strokes. Infarction leads to edema which can then compress the fourth ventricle and cerebral aqueduct. If not recognized, hydrocephalus may develop, as in this case, leading to brain herniation and death. This is a neurosurgical emergency, requiring immediate decompression. |
